Ovarian apoplexy, ovarian pregnancy and the IUCD

      Abstract

      This ovarian pregnancy, in a 35-yr-old gravida V para IV, is the 15th to be recorded in IUCD users who, within the past 5 years, have scored about 10% of all recognized ovarian pregnancies recorded in English. Because of confusion with so-called ‘ovarian apoplexy’ — i.e. rupture of a supposedly unimpregnated corpus luteum causing excessive intraperitoneal hemorrhage — early intrafollicular eccyeses may perhaps often go unrecognized. However, accidents attending IUCD use attract special attention; specimens of ovarian tissue from IUCD users with ‘ovarian apoplexy’ may thus be examined with particular care, resulting in increased detection of ovarian pregnancy in such patients. Alternatively, IUCD use may promote ovarian implantation by interfering with tubal function. Until a connection between IUCD use and ovarian pregnancy can be confirmed or categorically denied, reports of ovarian pregnancy in IUCD users should be made available to the medical community.
